Please fill out this form and meet with the Graduate Student Coordinator after registering for your final certificate course. The Graduate Student Coordinator will verify that you have met all of the certificate requirements and notify the graduation office. A certificate will be mailed to the address in your CIS account within about 2 months after the end of the semester you completed the requirements.
